COVID-19 Vaccine Updates
AFM is administering COVID-19 vaccines to all patients 18 years of age and over but we continue to focus on high-risk patients. Patients will be contacted as supplies become available.

- Make sure you have a My Health Connection account. If you do not have an account, create one here.
- Contact your Local Public Health Agencies with COVID-19 questions.

With more than a dozen locations throughout Northern Colorado, including urgent care options, Associates in Family Medicine is available when and where you need us.
We believe that a close relationship with a family physician is important in maintaining your family’s health. Each of our providers has been specially trained and board certified, where applicable, to provide comprehensive family medicine. The practice of family medicine includes pursuing ongoing education and training in health promotion and maintenance, disease prevention, counseling, patient education, and diagnosis and treatment of acute and chronic illnesses.
Visit with one of our expert neighborhood providers using your computer, smartphone, or tablet. No need to leave home, spend time in the car, or sit in a waiting room. Virtual Visits are offered across all AFM clinics, Monday through Friday, 7 a.m. to 8 p.m., and Saturday through Sunday, 8 a.m. to 8 p.m. Set up a Virtual Visit for:
- Non-emergent urgent care services
- Primary care appointments
- New patient appointments
For immediate care without an appointment, AFM Urgent Care clinics provide a convenient solution. Onsite services include care for acute injuries and illnesses—such as cuts, sports injuries, flu, or as an occasional alternative to a scheduled office visit—when it just can’t wait.
Caring for newborns, infants, children, and adolescents is an important part of providing comprehensive family medical care. All of our neighborhood clinics provide full pediatric services. Our physicians visit newborns and new parents in Poudre Valley Hospital in Fort Collins and Medical Center of the Rockies in Loveland. Children requiring hospitalization are also cared for by our staff of family physicians.
Did you know that our physicians perform hundreds of deliveries at Poudre Valley Hospital and Medical Center of the Rockies every year? We specialize in providing personalized care for the entire pregnancy, from the first prenatal visit to the hospital delivery.
Associates in Family Medicine also offers a number of additional services, including but not limited to gynecology procedures, dermatology, osteopathy, and mammography and digital imaging.
