By focusing on preventative care and regular health check-ups, our providers not only help you manage your current conditions, but also catch other health issues early on. We recommend the following preventive care for all of our senior patients.

  • Screening for colon cancer per current recommendations
  • Bone density test for women
  • Abdominal aortic aneurysm screening for men if history of smoking
  • Immunizations for pneumonia, influenza, and tetanus

Onsite senior health care

Our Community Care Team is a designated group of medical professionals and care coordinators who visit assisted living and skilled nursing facilities in Northern Colorado every week. They provide coordinated, onsite care to seniors living at these facilities to ensure continuity of care for patients’ physical, mental, and emotional health.
