When to schedule online

Existing patients can schedule an appointment online anytime through the My Health Connection portal. Use online scheduling for a variety of preventative and acute appointment types, including:

  • Adult annual exams
  • Well-child visits
  • Return/follow-up patient visits
  • Virtual Visits
  • Return OB visits and post-partum appointments
  • Returning sports medicine appointments
  • Chronic/ongoing care
  • Medication management
  • Paperwork
  • Mental health visits

Same-day acute visits for concerns such as:

  • Cold/flu
  • Fever
  • Vomiting
  • New pain or injury

Your appointment has an arrival time that allows you to register and get to your appointment room. Please be at the clinic at your designated arrival time. If you arrive past the appointment time, you may be asked to reschedule to allow for a full, complete visit with your provider.

If you have a severe or life-threatening illness, major trauma, or injuries (including chest pain, loss of consciousness, or uncontrolled bleeding), please call 911 or visit an emergency room immediately.

When to schedule by phone

If you are a new patient to AFM, please call our new patient line at (970) 237- 8069 to schedule your first appointment. If you are an existing patient, please call your AFM clinic to schedule an appointment for any of the following conditions or concerns and ensure you are scheduled accurately:

  • First prenatal visit, or OB intake visit
  • Nurse visits
  • Appointments with care team members
  • Procedures
    • Mole removal
    • Wart removal
    • IUD insertion or removal
    • Nexplanon insertion or removal
    • OMT services
    • Mammography (contact Horsetooth or Windsor)
    • DEXA scans
    • Colposcopies
    • Loop electrosurgical excision procedures (LEEPs)
    • Vasectomies

If you are experiencing any of the following symptoms, call us immediately to ensure you get the appropriate care you need:

  • Chest tightness
  • Shortness of breath or difficulty breathing
  • Any numbness, tingling
  • Dizziness or blurry vision
  • Severe abdominal pain

If you have a severe or life-threatening illness, major trauma, or injuries (including chest pain, loss of consciousness, or uncontrolled bleeding), or sudden onset of any of the symptoms above, please call 911 or visit an emergency room immediately.

Log in to My Health Connection

Current AFM patients can log in or create an account on their browser or through the app.

If you are new to AFM, please call our new patient line:
(970) 237- 8069.

Find your provider

  1. Under Your Menu, click Schedule appointment
  2. Select Provider Appointment
  3. Select your provider

Select your appointment type

Choose the appointment category that best fits your care needs.

Select a date and time

Review the available dates and times, and select what works best for you. If your provider is not available at a time that you need, click Who else could I see sooner? for other providers available at the same AFM location.

Confirm your appointment

Confirm your contact information and insurance, as well as any concerns that we need to address during your appointment. Select Confirm Scheduling, and you are ready to go!

How can I sign up for a My Health Connection account?

You may register for a My Health Connection account here.

What if I schedule the wrong appointment?

AFM has a review system in place in which one of our staff members will review appointments made online and make changes as necessary. If your appointment is changed, you will receive a call from us and a notification on My Health Connection.

Can I schedule an appointment for my child?

Parents or guardians may schedule an appointment for a child that is 13 years old or younger through proxy access. If a child is 14 years old, they will need to create their own My Health Connection account and schedule their appointments via their account.

To gain proxy access for children 13 years old or younger, log in to My Health Connection and select Request account access under Your Menu. Complete the appropriate form under Request access to child’s account.

Can I schedule an appointment for my child?

Who can I schedule with?

You may schedule with your primary care provider or any provider that you have seen in the last three years. If the provider you select does not have availability that meets your needs, you may also view providers’ schedules that work at the same AFM clinic location as the provider you originally selected.

What is an established patient?

An established or existing patient is anyone who has been seen in primary care within the last three years at AFM. If a patient has only been seen at an AFM Urgent Care, they will likely not be able to schedule online.

Can I schedule online via the AFM website?

You may access the My Health Connection portal from the AFM website. You must have a My Health Connection account to schedule online.

I am new to AFM. Can I schedule my first appointment online?

New patients must call to schedule their first appointment in order to be scheduled accurately and establish care at AFM. If this is your first appointment with AFM, please call our new patient line at (970) 237- 8069 to get scheduled. After you’ve completed your first visit with us, you will have the ability to schedule future visits online.

What if I can’t find an appointment that works for my schedule?

If you do not see any appointments that fit your needs, you may always call your AFM clinic for assistance in receiving the appointment time you need. You may also schedule an appointment and add yourself to a waitlist in the event a sooner appointment becomes available. AFM has three urgent care locations available for walk-in services if your concern requires immediate attention.

What happens when I add myself to the waitlist?

When you add yourself to the waitlist after scheduling an appointment, you may receive an appointment invitation via your My Health Connection offering you an appointment that is sooner than your original one. You have two hours to accept this invitation, and these appointments are filled on a first-come, first-serve basis.

Can I schedule imaging with AFM online?

Not at this time. If you require an X-ray, mammogram, or other imaging services, please call us to schedule or be directed to the appropriate location.

What if I need to be seen for multiple concerns and I don’t know what type of visit I require?

Decide which concern needs to be addressed first, and select the appointment type that best fits your needs. Your provider will work with you to address as many concerns as possible during your appointment time. They will also schedule a follow-up visit with you if needed.

I have proxy access over my child’s account. Can I schedule their appointments?

If your child is 13 years of age or younger, you can schedule their appointments through proxy access on My Health Connection. Just select their name in your account and schedule as you would schedule for yourself. If your child is 14 years old or older, you will not have the ability to schedule appointments under your proxy access due to Colorado law. Your child will need to create a new, personal My Health Connection account from which to schedule appointments.

I do not have proxy access over my child’s account. Can I schedule their appointments?

If your child is 13 years old or younger, you will need to obtain proxy access over their My Health Connection account to schedule appointments for them. Please do not attempt to schedule an appointment for your child under your name. Children 14 years old and older must have their own personal My Health Connection account and cannot be scheduled via proxy access.

Can I schedule a virtual urgent care visit?

Not at this time. However, our providers have availability in their schedules for Virtual Visits every day. We highly recommend looking at our providers’ schedules for your Virtual Visit needs to maintain continuity of care.

I’ve only been seen at AFM Urgent Care. Can I schedule an appointment online?

Patients who have only been seen at an AFM Urgent Care need to call our new patient line at (970) 237- 8069 or call an AFM clinic to schedule their first visit with primary care. After you’ve completed that initial appointment, you will have the ability to schedule future visits online.

My PCP is not correct in My Health Connection. Can I change this myself?

Please call your AFM clinic to request this change.

I am newly pregnant and want to schedule my first OB appointment. Can I schedule this online?

Please call your AFM clinic to schedule your first pregnancy appointment or OB intake visit. After your initial appointment, your other prenatal appointments may be scheduled online.

I received an invitation to schedule an appointment sooner after I placed myself on the waitlist. When I went to accept it, it stated the appointment was unavailable. Why?

These appointment invitations are on a first-come, first-serve basis. If the system states that the appointment time is unavailable, another patient accepted the invitation before you were able to do so. You will not be removed from the waitlist and are eligible to receive additional invitations if appointments become available.

Can I schedule using the app?

Yes. You can schedule online using the UCHealth app or using the My Health Connection portal on your web browser.
