Have no fear, back-to-school is here and AFM is ready to assist!
Make sure your child is ready for the first day of school by visiting a well-child clinic. Summer Well child clinics will be held on Saturday, July 27 and Saturday, August 3 from 9 a.m. to 5 p.m. AFM’s CSU office, Horsetooth office and Windsor office will be hosting all well child clinics.

CSU office, 9 a.m. – 5 p.m.
Horsetooth office, 9 a.m. – 5 p.m.
Windsor office, 9 a.m. – 5 p.m.
Saturday, August 3

What to expect at a well child visit:
Your child will receive a comprehensive wellness exam, including recommended immunizations and answering any questions you might have for the year ahead.
The provider will record and discuss your child’s development and growth measurements.
Yearly required forms such as sports physicals, daycare forms and growth and development charting can be completed.
