How to schedule

As part of the Village Medical family, we’ve changed our patient portal and record keeping system.

Existing patients: Your profile will be moved automatically to the new Village Medical Patient Portal based on your provider’s location.

New patients: Get started by calling your local clinic to schedule an appointment.

What can I do on the Village Medical Patient Portal?

Login to the Village Medical Patient Portal to:

  • Schedule an appointment
  • Attend your virtual visit
  • Request medication refills
  • View test results
  • Access your health records*
  • Pay your bill**
  • Chat with your care team

Starting July 18, 2023, all patients will use the Village Medical Patient Portal to schedule.

*If you’d like to access your prior medical records, use the My Health Connection portal.
**All bills pre-dating the transition to the Village Medical Patient Portal will remain in My Health Connection and should be paid using the My Health Connection portal.

What can I schedule online?

Existing patients can schedule an appointment online anytime for preventative and acute appointments, including:

  • Adult annual exams
  • Well-child visits
  • Return/follow-up patient visits
  • Virtual Visits (only available through the Village Medical Patient Portal)
  • Return OB visits and post-partum appointments
  • Returning sports medicine appointments
  • Chronic/ongoing care
  • Medication management
  • Paperwork
  • Mental health visits

Same-day acute visits for concerns such as:

  • Cold/flu
  • Fever
  • Vomiting
  • New pain or injury

If you require immediate non-emergency care, see our urgent care locations.

If you have a severe or life-threatening illness, major trauma, or injuries (including chest pain, loss of consciousness, or uncontrolled bleeding), please call 911 or visit an emergency room immediately.

What cannot be scheduled online?

If you are a new patient, call one of our clinics to schedule your first appointment.

If you are an existing patient, please call your preferred clinic to schedule an appointment for any of the following conditions or concerns and ensure you are scheduled accurately:

  • First prenatal visit, or OB intake visit
  • Nurse visits
  • Appointments with care team members
  • Procedures
    • Mole removal
    • Wart removal
    • IUD insertion or removal
    • Nexplanon insertion or removal
    • OMT services
    • Mammography (contact Horsetooth or Windsor)
    • DEXA scans
    • Colposcopies
    • Loop electrosurgical excision procedures (LEEPs)
    • Vasectomies

What will I have to do to get my information transferred to the new portal?

You do not have to do anything to have your information transferred to the Village Medical portal. This is handled by your care team. Please note that you will not be able to view historical health data or test results in the Village Medical Portal. That information is still available to you in your My Health Connection account.

Will my provider be able to access information from other doctors’ offices or hospitals?

Yes. Test results and notes will be available to your primary care provider (PCP) and any other healthcare organization through the State Health Information Exchange. You will not have to fill out any additional forms for this information to be sent to your PCP.

Can I message my provider?

Yes, you will still be able to message your provider and care team with questions about medical conditions, lab tests, scheduling, prescription refills, or any other matter. In order to contact your care team, please be sure to sign up for both the Village Medical Patient Portal and mobile app. Learn more at villagemedical.com.

Will I be able to see my messages and appointment history?

You will continue to be able to message your provider, schedule an appointment, and pay your bill any time of day with the Village Medical patient portal and mobile app. You will not be able to see messages or appointment history prior to moving to the portal, but rest assured your provider still has this information.

How do I access my child’s health information(Proxy Access)?

To view your minor child’s account and health information, you will need to add them to your Family Settings. You can do this by clicking on “My Profile” then “Family Access” from the menu options. From there you can add your child as a patient and verify their identity.

What is the difference between the Village Medical portal and mobile app?

The same information is on the Village Medical patient portal and the Village Medical mobile app, but you will need a separate login for each. The patient portal is accessed via a computer/browser and has more functionality. The mobile app is formatted to better suit your phone. Accounts for children under 13 are not available on the mobile app.

What can I schedule online?

You can schedule annual wellness visits (AWVs)/annual physicals, problem or sick visits, follow-up appointments, pediatric visits, sports physicals, surgical clearance, and Welcome to Medicare visits. Visit afmnoco.com, villagemedical.com, or log in to your patient portal to schedule online.

Who can I schedule with online?

You can schedule an appointment with your primary care provider, a member of their care team, or any other Village Medical provider in your area.

Where do I pay my bills?

You can continue to pay any bill received before your office transitioned to Athena here. If/when you are billed after moving to the new patient portal, you can pay your bill by logging into the Village Medial patient portal or by calling 844-969-0686.
