At Associates in Family Medicine, we’re investing in a vibrant Windsor – a community where our patients and our families can continue to live, work, and play with passion and vitality. After all, we’ve been serving the town of Windsor for the past 12 years, and it’s a place many of our providers and staff members call home.

That’s why in Spring 2019, we’re moving to a brand-new facility in the heart of Windsor. From the corner of Main Street and 17th Street, our two-story, 14,200-square-foot building will offer a new urgent care clinic while also expanding our existing primary care services – helping us better meet the needs of patients and stand behind our local community.

Why is AFM building a new facility in Windsor?
AFM has been caring for patients in Windsor for over a decade. It is our community, and we want to serve it the best we can, especially in light of new developments and growth. By building a larger, state-of-the-art facility in the heart of town, we can expand our urgent care and primary care services – helping us provide the highest level of accessible, compassionate, effective healthcare to our Windsor patients, neighbors, family members, and friends.
Is AFM opening a second location in Windsor with this new facility? Will the original office remain operational?
No, AFM is not opening a second location in Windsor. We are building a larger, state-of-the-art facility to be able to expand our primary and urgent care services within the community. Our staff and providers from the existing Windsor office, along with a number of new additions to the AFM team, will be moving to the new building and serving patients from a more expansive, purposeful space.
Where will the new facility be located? How far is it from the current Windsor Office?
The new building will be located on the southeast corner of Main Street and 17th Street, just a half mile west of our current Windsor Office.
What features/services does the new location offer that the old Windsor Office did not?
The new facility will provide expanded urgent care services for non-emergent, immediate healthcare needs. Also, with over five times the square feet of our existing location, it will deliver more expansive primary care services and accommodate more patients. Lastly, we are excited to offer onsite X-ray services at our new facility.
When will I be able to be seen at the new location?

Do I have to call to make an appointment, or can I walk in?
Our new Windsor facility will include a dedicated Urgent Care clinic for immediate, acute care of non-emergent injuries, illnesses, and conditions. Urgent Care patients will be seen on a walk-in basis. However, an appointment will be required for all primary care visits.
What can I expect as a new primary care patient?
New patients at our new Windsor facility will be greeted by our reception staff and provided with the appropriate registration and medical history forms to complete. We will ask you to arrive 15 minutes before your scheduled appointment to complete your paperwork. You will need to bring a current list of your medications, your insurance card or a copy of your card, a method of payment if your insurance requires a co-pay or deductible, and a list of questions for your provider to ensure you get the answers you need.
Do I need to be an AFM primary care patient to visit the Urgent Care?
No, you do not need to be an AFM primary care patient to be seen at our Urgent Care clinic. We welcome both new and existing patients. If you have not been seen at an AFM office or Urgent Care in the past, simply have your in-network insurance information in hand and a method of payment as needed.
Will you still have all of my medical records and insurance information on file if I’m an existing Windsor patient?
Yes. All of our AFM offices use the leading electronic medical records (EMR) solution in the Northern Colorado region: Epic. Using this system allows us to easily, electronically, and securely access patient medical records and ensure absolute continuity of care – no matter the location. In addition to utilizing Epic, our new Windsor facility will continue to offer My Health Connection as a convenient option for our patients. What types of physicians and providers will you have?
At our new Windsor facility, patients can choose to see a MD, PA, or nurse practitioner for their primary care needs. All of our providers are board certified.
If I’m a patient at the existing Windsor Office, will my same primary care doctor be working out of the new building?
Yes. All of our current Windsor providers will work out of the new facility upon its opening in the spring of 2019.
Will you see infants and children at the new location?
Yes. Our new facility will offer the full range of pediatric services to keep your little ones healthy.
Will you provide complete maternity care at the new location?
Yes. Our obstetrics services will include complete prenatal, labor and delivery (offsite), and postnatal care.
If my regular doctor is unavailable, can I see someone else?
Of course! In the event that your primary doctor is unavailable to see you as soon as you require, we encourage you to make an appointment with another available provider on our team. Or utilize the walk-in Urgent Care clinic for your acute care needs.
Where will the new facility be located? How far is it from the current Windsor Office?

Are any of your doctors accepting new patients?
Yes. We have a number of providers who will be accepting new patients once our new facility opens in the spring of 2019. We are also accepting new patients at our current Windsor location. Call (970) 686-0124 to schedule your appointment.
Will I be able to get lab tests or X-rays done at the new facility?
Yes! We are excited to offer onsite radiology services at our new facility to build on our high-quality care for patients. Our new location will also provide onsite laboratory and point-of-care testing – making it a convenient, “one-stop” location for your various healthcare needs.
Will I be able to get my prescriptions at the new facility?
For added convenience and a higher level of care, our new location in Windsor will offer onsite pharmacy services – giving our patients immediate access to reasonably priced prescriptions for several generic one-time medications.
What insurance plans will be accepted at the new location?
Our new Windsor location will accept the same insurance plans as each of our other locations across Northern Colorado. We recommend that you contact your insurance provider to confirm whether AFM and our providers are in network for your plan, as well as to clarify any out-of-pocket costs for specific care services. AFM accepts most major insurance and managed care plans offered locally, including:

Will you be able to refer me to a specialist or other provider if needed?
Absolutely! By integrating our urgent and primary care services and using the leading electronic medical records (EMR) system in our region, we will be able to easily access your patient records and connect you with specific health specialists or services that meet your ongoing needs.
When should I visit the Urgent Care at your new facility rather than make an appointment with a provider?
Visit our new Urgent Care clinic if you are dealing with a non-life-threatening condition and your primary care provider is not immediately available. These types of conditions include:

  • Common colds and coughs
  • Small cuts, bruises, or burns
  • Fevers and flu
  • Ear infections
  • Sinus infections
  • Sore throats
  • Bronchitis
  • Minor drug reactions
  • Urinary tract infections
  • Mild diarrhea or vomiting
  • Sprains
  • Minor broken bones
  • Sores and blisters
  • Mild abdominal pain
  • Muscle, joint, and back pain
